USA Patriot Act / Customer Identification Notice

Verification of the information provided by the customer will be conducted prior to opening the account. The procedure applies to any customer who opens an account with us.

Collection of Information

The following information will be collected from the consumer prior to opening account with us. We may collect additional information as it deems necessary to aid in account administration (e.g., phone number, mailing address, mother’s maiden name, etc.):

Name
Date of Birth
Residential address
- For individuals not having a residential address, an Army Post Office (APO) box, Fleet Post Office (FPO) box, or street address for next of kin may be used.
Taxpayer Identification Number
- For U.S. Individuals, this shall include a Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN).
- For Non-U.S. Individuals, this shall include one or more of; a taxpayer identification number, passport number with issuing country, alien identification card number, or any government issued identification with number and issuing country, which evidences residence or nationality and bears a photo or similar safeguard.
